As with all medical practices, new patients will be required to register with us. We collect and store your information according to privacy regulations.
On your first visit we would require you to complete and sign a registration form containing your relevant personal details and an agreement related to accounts that you may receive from our practice.
You may download our registration form in PDF format. If you have a PDF application installed on your PC / Mac, you may fill in and sign the form electronically. Email the completed and signed form to us at admin@drbothma.co.za.
Alternatively, print the form, fill it in and bring it with you on the day of your consultation.

We adhere to the Patient Confidentiality Rules of the Health Professions Council of South Africa as prescribed for ethical medical practice.
The Protection Of Personal Information Act No4 of 2013 (POPI) compels us, by law, to collect, store and protect your personal and medical information according to regulation.
When you register with our practice, we will require that you acquaint yourself with and sign an informed consent agreement specifying the details of the collection, storage and disclosure / sharing of such information - as prescribed by the legislation.

We issue prescriptions for medication and assistive devices / orthotics directly related to your current case management and peri-operatively, where surgery is involved.
Prescriptions for management of co-morbidities and other chronic conditions are the responsibility of your treating family doctor or relevant specialist.
When required, we will refer you to an appropriate healthcare practitioner (colleague specialist, physiotherapist, biokineticist, dietician etc) for assessment, treatment or rehabilative purposes.

Consultations with Dr Bothma usually take from 30 minutes to 45 minutes. Complex issues may take up to an hour. We schedule new consultations for 45minutes and follow up consultations for 15 minutes.
The consultation includes a thorough enquiry and physical examination, assessment of special investigations, detailed explanation of the clinical findings (diagnosis) and discussions on treatment options.
We may require that you undergo X-rays, ultrasound studies, scans and/or blood tests prior to the consultation. We will make the arrangements for the investigations on your behalf and will let you know when and where you have to be, to have the tests done.

In the event that you are not able to keep your appointment with us, whether by cancellation or postponement, we kindly request that you notify us at least 48 hours before the scheduled appointment by calling us on 011-704-6826.
We accept that emergencies or urgent circumstances may arise, where you may have to cancel or postpone your appointment at short notice. Please call us to discuss and re-schedule.
Please diarise your appointment correctly. We will contact you or send you a reminder SMS in order to avoid disappointment and disruption of our schedule.

Our practice charges Private Rates. Consultations are payable on completion. Procedures are payable on day-of-discharge from Hospital when a 15% discount becomes applicable.
We issue detailed invoices and paid-up statements that you may submit to your health insurer (medical scheme) for re-imbursement.
When a procedure is scheduled, we will e-mail you the surgical fee cost estimate with the appropriate coding and personal / surgical information that you may submit to your health insurer when you obtain authorisation for funding.
We accept Visa & MasterCard Credit Cards, Cash and EFT.
